Hahaha - reminds me of something that happened at the vet clinic years ago. On the schedule the receptionist entered the purpose for an appointment as "Check Tear Duck" (it's supposed to be tear duct)... someone else apparently saw that it was wrong and crossed it out and rewrote underneath... "ducked"
